Look carefully at Table 10-5 and you can see that I list several interactions for cimetidine and none at all for the other three H2 blockers. Why? Because cimetidine’s chemical structure enables it to lock onto and inactivate several enzymes your body needs to metabolize various other drugs. Famotidine, nizatidine, and ranitidine don’t behave this way.

Look again. You may notice many more interactions for omeprazole than for the other four PPIs. For the moment, just accept the fact that although the PPIs have similar chemical structures, omeprazole seems more likely to interact with other drugs. On the other hand, all the current studies show the five PPIs are similar in safety and effectiveness, which means that good old omeprazole may get blamed for more side effects just because it has been around longer.
